Youth Hostel Outreach

Overview

The Youth Hostel Outreach Program (YHOP) is a relationship based mental health outreach program for young adults who are homeless/marginally housed and/or living in the youth shelter system. The program provides assessment, intense short term case management, support and informal counselling to young adults experiencing significant challenges with their mental health posed by social, emotional and psychiatric challenges as well as those imposed by societal oppressions. Using a recovery approach, the program aims to connect and navigate youth and young adults to community resources, to secure stable housing, and enhance mental health functioning.

Eligibility

Youth and young adults 16-24 years of age, must be homeless or marginally housed in the city of Toronto, and experiencing mental health issues that impact their daily functioning.

How Do I Access This Service?

Referrals for this program are generally made through the various youth shelters in Toronto. Please complete the New Outlook Referral Form.
